You don't need to know. Pick the closest institute — or 'not sure' — and the board routes it. Most cases touch more than one institute anyway; they're read as one case.
I've been told nothing more can be done.
That usually means nothing more can be done there. Four institutes re-read the full case together and return a written second opinion — agree or disagree, with reasons.
I can't travel right now.
You don't travel to start. Records are read remotely, and a medically-trained case-manager plans travel only if — and when — treatment justifies it.

What if my case is not a fit?
The plan says so. We say no when a case does not fit.
Do you treat cancer, heart disease or transplant cases?
No. We concentrate on the functional and age-related-decline axis. Oncology, cardiology and transplantation are out of scope; we refer to partner centres for those.
I'm a doctor referring a patient.
Send the case the same way. The board returns a written, named clinical opinion — sent to the patient and, where requested, to the referring physician.
